Steps to proceed to take Birth or Death certificate.

• Get the application form and Chelan from Citizen charter (Download Birth/ Death application form).
• Fill application form and Chelan.
• In Public health department get an initial of any one of the health assistants on the application form.
• At Manager room get a date stamp on the application form.
• Pay the amount in the counter at Accounts section.
• Submit the application form at citizen charter and take the receipt.
• To issue of the certificates nearly 10 working days are required.

Important points to note

• Up to one year name of the child will be included by the request of any one of the parent without any fee.
• Up to 15 years with a fee of Rs. 12 name will be included by the request of any one of the parent.
• After 15 years we don't have any provision to include the name of the child in our records as per 1969 Act.
• Name spelling corrections and omission of middle name or last name etc will be corrected with the application of the any one of the parent with notary on Rs.10 stamp along with declaration form duly signed by two gazetted officers.
• In case of Birth or Death occurred at home must be registered within 21 days.
• After 21 days and before 1 year Birth or Death will be entered into office records with submission of notary on Rs.10 Stamp.
• After 1 year, Birth or Death will be entered into office records with the permission given by RDO, Narasapuram.

• For Name inclusion : Rs.12
• For One certificate Rs. 20 ( there after Rs.10 for each certificate)

Fee Particulars of Death certificate.

• For One certificate Rs. 20 ( there after Rs.10 for each certificate)

Issue of Non Availability certificate

The following are needed to obtain NOC.

• Grandparent declaration form
• Xerox copy of Ration card
• Xerox copy of School Study Certificate.
• Notary on Rs.10 Stamp.
